ONLINE SCHEDULING AND MSP-COVERED TELEHEALTH (VIDEO AND PHONE) VISITS NOW AVAILABLE!

During the COVID - 19 pandemic, to keep our communities as safe as possible through physical distancing measures, all appointments will be scheduled as Telehealth visits first.  During your Telehealth visit, you and your physician may agree to an in-person visit which will be promptly arranged. When the Provincial Health Officer (Dr. Bonnie Henry) indicates it is safe to relax physical distancing measures in our clinic, you will also be able to use our online scheduling system to schedule an in - person visit.

In order to comply with College and MSP/Government expectations, the patient must be present on the telephone call or video call.  (i.e. you can not speak on behalf of another person, unless that person is virtually or physically present.)  We can help you accommodate this by doing a three way call.  This includes all patients, including babies, children and the elderly.  

Please read through this page before booking an online appointment. There are links on the bottom of this page to proceed to telehealth booking.


Checklist for online scheduling for telehealth appointments (please read this before booking):

1.     Consent form: If you have not already done so, please click here to request a secure consent form to sign electronically. You can  schedule an online appointment after emailing us even if you have not returned the consent form yet.

2.     Patient information: You must be an existing patient of Collingwood Medical Clinic (our physicians are currently not accepting new patients).  You will need to enter your date of birth and BC Personal Health Number (PHN). Your PHN can be found on the back of your BC Services Card or your BC driver’s license.

3.     Reason for your visit: Please enter as much information as possible (limit 50 characters).  If you are scheduling a video appointment, please indicate the best phone number to call you at if there are technical difficulties.  If you are scheduling a phone appointment, you can confirm your phone number in the next two steps.

4.     Confirm displayed contact information: Confirm your current email address and phone number(s).  

5.     Select either a video or phone appointment using the appropriate tab.  All doctors are currently offering phone and video appointments.  Please note that appointment times are approximate and your physician may call from a “blocked,” “private,” "unknown," or otherwise unfamiliar number.

6.     Appointment confirmation: Immediately after scheduling the appointment, check your email for a confirmation email and if applicable, test the link to confirm your ability to connect using video.  Please note that to reschedule a video appointment, you must use the link below. 

7.     Photos & Forms: If you have photos or a form for your appointment, please email them to us after you have booked your appointment following the above steps. Our email address is reception@collingwoodmedical.ca. You DO need an appointment for any form you wish a physician to fill out. If it is a drivers form, please call our office as you will need an in-person appointment for this type of form.

Please be aware that just like online shopping or email, virtual care has some inherent privacy and security risks that your health information maybe intercepted or unintentionally disclosed. We want to make sure you understand this before you book your virtual appointment.  In order to improve privacy and confidentiality, you should also take steps to participate in this virtual care encounter in a private setting and should not use an employer’s or someone else’s computer/device as they may be able to access your information.  If you want more information, please contact us at our office. If it is determined you require a physical exam during the virtual appointment you may still need to be assessed in person. You should also understand that virtual care is not a substitute for attending the Emergency Department if urgent care is needed.

In-Person Appointment Booking

In-person appointments are available on a limited basis. Please read the top of this page regarding the Covid-19 pandemic for more information on why in-person visits are limited. Drs. Leung,  Dr. Lim, Dr. Hanam, Dr. Niou, and Dr. Liu's patients can book in person visits online at this time.

For any other in-person appointment booking, you will need to call our clinic and speak to a receptionist. Our phone number is 604-435-3388. Please be advised that our phone hours are Monday - Friday, 8:30 am - 12:00 pm and 1:00 pm - 4:00 pm (closed for all statutory holidays). Please do not leave a voicemail as it will not be followed up.

Dr. Tong has reduced her hours in 2022 temporarily. During this time, Dr. Jeraldine Chan or other locum physicians are available to care for her patients. To book an appointment on-line click here.

Q: I did not get a link for the appointment. What do I do now?

If you did not get a link, please check your junk mail box.  In any event, the doctor will call you near the time of the appointment should the link not be working.  As such, please provide us with your up to date contact information by clicking here.  

Q. I’ve been trying to book an appointment online and the site would not let me.  What do I do now? 

To book an appointment at Collingwood Medical Clinic, you must be an existing patient. 

If you are an existing patient at Collingwood Medical Clinic and you cannot book online, it could be that your personal information (i.e. Personal Health Number and/or date of birth) is not matching our database.  If that happens, please call our office at 604-435-3388 to make sure we have your correct personal information. 

Q. I have an upcoming appointment but I did not receive a reminder. Do I still have an appointment?

If you booked an appointment on-line, a reminder will get sent out to you 48 hours prior to your appointment time.

This booking page is ONLY for the active patients of Collingwood Medical Clinic. If you are not our patient, we will not be able to schedule an appointment for you. As Dr Golin's practice is closed effective April 30, 2022, Dr Golin's patients are no longer able to schedule visits online.
